Holders of this account agree to pay all collections and legal fees required to collect payment as well as interest charges at a rate of 1.5% per month on any delinquent accounts.

Holder(s) of account accept full responsibility for all charges made to this account, according to the instructions set forth above. Instructions may be changed by providing written notice to Model Cleaners and will be recognized upon receipt of signed notice.
I authorize Model Cleaners to keep my signature on file and charge my credit card for weekly charges incurred for dry cleaning/laundry services.
Consent to Clean: From time to time you may ask us to clean an item that we may identify as “at-risk” if processed due to its color, fabric content, buttons or ornamentation. To save time and ensure that there are no delays in processing your order, you hereby grant your consent for Model Cleaners to use their best efforts to process these items and understand damage may occur and release Model from any liability.
